You don't need to fill out anything in this section now. But it's important that you have the concluding clauses below in your text file for later.
You must swear or affirm that the contents of the affidavit are true by signing your affidavit in front of a commissioner for taking affidavits. This could be a lawyer or notary.
If you so swear, you'll sign the affidavit in front of the commissioner on the line at the bottom right. Print or type your name underneath that line.
The commissioner will complete the rest of the clauses. They'll note the location (the city) and date of the signing. They'll also sign the affidavit themselves.
- Click the Open text version button.
- Copy and paste the text into the word processing document where you've pasted your affidavit contents from the previous sections of this guide.
- Format the concluding clauses to look like they do below. The formatting will not transfer to your text version. Make sure the line where you'll sign your name is aligned to the right-hand side, like below.
